This is for our regular/ funded patients who have met one of our doctors in person and completed our enrolment form.
Please fill out this form carefully. Once we have received it, any changes or additions will incur extra cost
It may take up to 48 hours to process your request Monday to Friday, and any requests that come in during the weekend will be processed on Monday.
You agree to pay MEDIQUAL™ fees for services rendered.
You agree to pay any surcharges that your chemist may charge for the prescription, as this is out of our control.
No narcotics will be prescribed over the Internet.
Some prescriptions are funded to for only one month at a time. This is not in our control. We will advise you if this is the case.
The medication is intended only for the person prescribed, and strictly to be taken as directed. If you have any doubts about safety, your understanding of why you are taking it, or how you are taking it, please contact us.
This is a service for repeat prescriptions for ongoing management of a known medical condition that is stable and controlled. This is not a service for the management of acute illness or initiation of new medications. We reserve the right to refuse to prescribe if there are any uncertainties about your request or if we need to review you to ensure we give you best care and best practice.
